The Physical Activity for Health Officer role will provide a service to those with a chronic condition who wish to engage/re-engage in physical activity and may need additional support before participating in a current LSP programme
Overview
- Pilot Programme in 6 counties - Wexford, Waterford, Wicklow, Limerick, Mayo, Longford
- PAfHO will work in collaboration with Slaíntecare, the HSE, the local authority and other key stakeholders to provide opportunities for those with chronic conditions to get active.
- The Physical Activity for Health Officer initiative under this fund will aim to effectively address physical activity as a risk factor for chronic conditions.
Aims
- To accept signposting from health services initiatives, such as Chronic Disease Management Programme and Social Prescribing services and facilitating their enrolment in subsidised LSP or partner programmes for recreational sport.
- To co-ordinate and quality assure the delivery of a free at point of use, HSE approved, structured physical activity programme, with follow-up 3 months post programme completion, for people with chronic condition or long-term health condition who don’t require clinically led exercise programmes, with a focus on building their skills and confidence to engage with and maintain recreational sport participation independently.
- Through communication, engagement, and partnership, to increase healthcare practitioners’ awareness of physical activity/recreational sport opportunities that service users can be signposted to.
